If you would like your child to attend either of the Alpha Child Care facilities you will need to complete both the Application and the Privacy Policy Permission Form.
Steps to Complete
1. Download the form them to your computer / device
2. Fill out the forms
Signatures can either be done by downloading / printing / singing and uploading
